Connecting to SHARE for Patient-Centered Medical Home (PCMH)*

Connecting to SHARE helps your clinic qualify for Medicaid PCMH reimbursement. What you might not realize is that your single PCMH SHARE connection can also enable all the additional benefits and capabilities of SHARE, including saving time and money, increasing patient health, and working towards Meaningful Use Stage 2 requirements.
Through SHARE, your PCMH practice can:
- Exchange clinical patient information and make referrals securely, efficiently, accurately, and immediately in real time.
- Connect to hospitals, physicians, nurses, labs, public health registries, long-term care entities, and others involved in your patients' care.
- Receive notifications as soon as your patients are admitted to or discharged from the hospital.
- Avoid duplicate testing and procedures through records sharing with patients' other providers.
