The Lake Village Clinic PA in Lake Village Arkansas established in 1910 is now exchanging continuity of care documents (CCDs) with the State Health Alliance for Records Exchange (SHARE), which allows health care providers to seamlessly exchange patient medical information.
The secure online network acts as a statewide hub that electronically transfers secure and encrypted patient health information from provider to provider. Shared data includes patient demographics, allergies, laboratory data, prescription medications, admission, discharge, transfer summaries. Benefits of this level of collaboration include greater efficiencies for the hospitals and more effective care for the patients of Lake Village Clinic.
“SHARE has been able to show us new ways to derive more value from our investment in information technology,” said Suzanne Wright, Administrator with Lake Village Clinic. “With the expertise of the SHARE team, we are now customizing our system so that our clinical care teams and referral physicians will automatically receive notifications when we transmit patient health information to them.”
A connected, informed healthcare system is safer, more effective, and less costly overall than any other way of providing patient care. The only way to truly fulfill patient needs is by sharing data and giving all providers access to the same clinical data.
The clinics’ referral network care teams can now also view the patient data, as data is flowing from eClinical Works (eCW) interface to SHARE in real-time. Also, access to real-time and historical patient health information through the SHARE Virtual Health Record (VHR) includes:
- Admission, Discharge and Transfer Data, Allergies, Problems, Medications, Vitals, Social History
- Demographic, Insurance, and Past Provider Information, Laboratory Results, Radiology Reports, Pathology Reports
- Discharge Summaries, Continuity of Care Documents, Progress Notes and SDOH
In Arkansas, where many large and small health systems use disparate EHR systems, including in the Arkansas Delta region, this is a particularly important milestone because the single interface to SHARE will allow providers the clinic locations and staff to access laboratory results, radiology reports, progress notes, discharge summaries from outside hospitals and specialty clinics.
SHARE also assists Lake Village Clinic in meeting the Medicaid PCMH program metrics as well as the ARHP Clinically Integrated Network metrics such as tracking high-priority patients, following up with patients within 10 days and knowing high utilizers of the emergency department, whether it is at Chicot Memorial Medical Center, Arkansas Children’s Hospital, Ashley County Medical Center, CHI St. Vincent, McGehee Hospital, UAMS Health, JRMC or Baptist Health.
